原因不明の腰痛に対する対処法

腰痛には様々な種類がありますが、そのおおくは原因を特定されています。
しかし、稀に「原因不明」の腰痛があるのも事実です。自分でも様々な治療を試みてみたものの、さっぱり良くならない。病院にいってもなんだかわからない、といった状況も少なからずあると思います。

しかしながら、もしかしたらそれは腰の問題ではないかもしれません。レアケースではありますが、腰の筋肉や神経などではない腰痛もあるのです。

本記事では、原因不明とされる腰痛の様々なケースを紹介いたします。

ここでは腰痛を発生させる様々な事例をご紹介していこうと思います。腰痛は通常、ぎっくり腰やスポーツなどでの捻挫による「急性腰痛」や、腰の筋肉や神経などに問題をかかえる「慢性腰痛」にわけられます。急性腰痛をこじらせて慢性腰痛になってしまうこともあるので注意が必要です。

「腰痛の八割は原因不明」とすら言われるほどに多種多様な症状と原因をもつ腰痛ですが、もしよほどよくならないようであればその原因が「腰」ではない場所にある可能性もあります。腰痛の原因を突き止めるのはそれほどに難しいのです。

しかし、原因を不明だとされながら、思いがけない原因によって発生した腰痛を見つけ、治療できた事例もあります。ここでは、原因不明とされる腰痛の種類等を紹介します。

心因性腰痛

整形学会がしめした指針で、近年驚くべき報告がありました。

それは、「腰痛にはストレスがよくない」とされる研究結果です。心因性腰痛とよばれるこれが、実は腰痛の中でも脊椎等に損傷がないものの大半をしめているのではないかとすら言われています。ストレスを感じることで複数の筋肉が相互に深く関わっている腰にプレッシャーがかかり、激しい痛みを感じることになってしまうのです。
この心因性腰痛はめだった外傷等がないことから「非特異的腰痛」とも呼ばれます。職場の人間関係、仕事上の不満、鬱などの心理的な要因が関与しているのです。この場合の治療は、安静やマッサージ、脊椎牽引は必ずしも有効ではなく、できるだけ普段の活動をして、かるい運動をすると良いとされています。三ヶ月程度の運動で効果があるそうです。

心因性腰痛の治療は、まず腰部の痛みをとる薬物治療がよいとわれています。鎮痛薬に近いものが処方されますが、精神的、心理的要因を改善するために抗うつ剤が有効なケースが多数あります。抗うつ剤をのみはじめて二週間程度で改善したというケースもあります。

薬物治療と平行して、心理療法やカウンセリングなどを受けます。そちらは心療内科や精神科での治療を行う場合もあります。周囲の人の理解が得られないなどの状態もありますので、家族や周囲の人が助けてあげることも重要です。

緊張性筋炎症候群

TMS(緊張性筋炎症候群)とよばれるものも心因性腰痛の一種ですが、こちらはもう少し深刻で常時痛みを感じる上に、夜寝る前などにも痛み出します。精神的な緊張が炎症を引き起こしてしまう重篤な病気ですが、これもまた心療内科や抗うつ剤による治療が効果的とされています。

原因不明の腰痛を取り除くための心得

こうした心因性の腰痛には、外傷や炎症もなく、筋肉にも骨にも問題がない場合があるため、治療が難しい場合があります。薬物治療なども効果がありますが、以下の様なことに気をつけるようにするだけで症状が落ち着くことがあります。

気の持ちようを変える

腰痛は、痛みを何とかしようと思えば思うほど、痛みが増してくることが多いとされています。認知行動療法と呼ばれる治療法で、「考え方を変える」ことで痛みとの付き合い方を変えることが治療に効果的であることがあります。

それは、「痛いからなんとかしようと焦る」のではなく「ひどい痛みと、自分はここまで付き合ったのだ」と思うことです。痛みと付き合える自分を褒めてあげてください。

マットレスや布団を変える

睡眠環境の変更も治療に効果があります。なかでもマットレスがいつの間にか体に合わなくなっていて、そのせいで体に思わぬ負担をかけていたということもあるようです。体にあわない布団でねていると、不安を感じることが増えることがあり、それが腰痛となって現れることがあります。

靴を変える

靴を変えてみるだけで腰痛を抑えることができる場合があります。特に女性は靴の変更によって精神的な負担が和らぐことが知られています。おしゃれも良いですが、腰痛がひどい場合には自分にあった楽な靴を履いてみましょう。

息を抜く

息抜きをするようにしましょう。根を詰めすぎたり、多すぎる仕事などに押しつぶされたりしてしまう事がよくあると思いますが、あまり気にしないようにして、なにもかも忘れてリラックスする時間があると良いでしょう。

まとめ:原因不明の腰痛は心因性かも

原因不明の腰痛は、心因性のものかもしれません。

心のケアと腰のケアを両方行って、気持ちのよい毎日を過ごせるようにするとよいでしょう。
